To facilitate this informed consent, we have clients read a number of documents, give them an opportunity to ask questions and have them sign releases indicating their understanding. The material read are as follows:
“Information for Prospective Residents and Their Families”, a Dawn Farm brochure.
“Dawn Farm Rules and Regulations”, a Dawn Farm brochure.
“Know Your Rights”, a publication of the State of Michigan describing the legal rights of all recipients of substance abuse services, the release gives the name of the Recipient Rights Advisor.
“Dawn Farm Determination of Client Fees”.
"Belongings Release" describing our policy on storing client possessions.
"Dawn Farm Client Handbook" and "sign off" sheet including:
"Dawn Farm Welcome Statement", welcoming the client to the Dawn Farm program.
"Information about Dawn Farm programs" and how to access those programs
"Confidentiality of Alcohol and Drug Abuse Patient Records", describing how medical and drug and alcohol related information may be used and disclosed.
"Dawn Farm Statement of Client Rights and Responsibilities" details client rights and responsibilities specific to Dawn Farm.
"Program Restrictions" details that some of the client's rights will be restricted while in treatment.
"Dawn Farm Statement of Ethics for Clients" details what clients can expect from the program the staff ethically.
"Dawn Farm Client Complaint Procedures" details the client's options for addressing complaints and grievances.
"Discharge Policy", detailing the reasons a person might be discharged prematurely from Dawn Farm and how to appeal any such decision.
"Types of Discharge", describing the different ways a person may be discharge from the Dawn Farm program.
"Safety and Emergency Procedures", describing the basic safety procedures at Dawn Farm.
"Information about Tours and Photographs", describing the policies when a public group tours the farm and the use of photographs at Dawn Farm.
"Medication Use While at Dawn Farm", information regarding medication use and the policies of Dawn Farm programs.
Access to the communicable diseases binders, including TB and HIV/AIDS information, indicating that the client has received and read information on these diseases.
"Consent for Treatment", a release consenting for treatment to a specific Dawn Farm program. (Outpatient, Spera, DFD/Stony Creek)
"Client Information Release Authorization for Follow-up Contact after 72 hours", asking for permission to contact them after treatment and for the names of those who will know their whereabouts.
Releases that specifically give Dawn Farm permission to communicate with the pertinent members of the criminal justice system, family members and friends chose by the client and former treatment providers.
