CONTRIBUTION TO MISSION & CONTINUUM OF CARE
The goal of Dawn Farm is to remove the barriers that people with addictions face to entering into recovery. Dawn Farm Jail and CCOP staff further this mission by providing substance use treatment services to clients that are currently incarcerated or participating in Community Corrections Outpatient programming. Jail and CCOP staff also create aftercare plans with clients to reduce relapse or recidivism risk.
CORE RESPONSIBILITIES
Serves as primary liaison between Dawn Farm staff, Washtenaw County Jail, and Community Corrections staff
Participates on the Washtenaw County Reentry Team
Completes jail and CCOP level of care assessments
Coordinates jail diversion activities to transition incarcerated clients into Dawn Farm services
Facilitates group therapy sessions
Provides crisis intervention services
Provides integrated case management services
Maintains complete record of client's treatment
Collects and maintains record of payments, as needed
Completes referrals for substance testing throughout course of treatment
Interacts with other community agencies regarding treatment progress and referrals, as needed
Coordinates treatment resources for higher levels of care, as needed
Connects clients with other community resources for transportation, food, housing, etc. as needed
Participates in all required trainings, staff inservices, and special events
PROGRAM COORDINATOR CORE RESPONSIBILITIES
Develops programming for Community Corrections in consultation with Outpatient Coordinator and Clinical Director
Provides 24/7 on-call support for Community Corrections staff
Hires and oversees training of new Community Corrections staff
Provides regular supervision, facilitates regularly scheduled staff meetings, and completes annual evaluations for Community Corrections staff
Acts as part of the Dawn Farm administrative team by participating in admin meetings and activities
Compiles and maintains program data and other information for program evaluation
Oversees compliance with licensing, accreditation, and contract requirements, as applicable, in consultation with Outpatient Coordinator and Project Manager
Addresses complaints, as needed
Maintains relationships with other community providers
Participates in all required supervision, training, seminars and special events.
Completes other duties, as requested by Clinical Director or President

Able to respond to client's internal experience of fear when the client's external expression obfuscates their fear.
Able to maintain a positive and hopeful relationships in a jail setting.
ALL DAWN FARM EMPLOYEE RESPONSIBILITIES
Integrates Dawn Farm mission and core values into daily functions
Familiar with recovery principles and the philosophy and practice of 12-step recovery
Pursues continuing education to improve job knowledge and maintain licensing/certification, as applicable
Able to interact with a wide variety of people in a cordial, professional manner, both independently and as part of a team
Able to interact with clients and family members in an empathetic manner while maintaining professional boundaries
SUPERVISORY RESPONSIBILITIES
Supervises Jail and CCOP staff
CREDENTIALS
Master’s degree from an accredited program in social work or a related field
CAADC or plan for certification
CPR certification
RECOVERY
At least eighteen months of continuous sobriety, if in recovery from addiction.
If ever a client in any Dawn Farm program, at least one year since the date of last service.
