CONTRIBUTION TO MISSION & CONTINUUM OF CARE
The goal of Dawn Farm is to remove the barriers that people with addictions face to entering into recovery. The Outpatient Coordinator at Dawn Farm furthers this mission by overseeing all therapeutic programming for outpatient services.
CORE RESPONSIBILITIES
Oversees funding for treatment services (public-funding, self-pay records, speciality courts, etc)
Interacts with other community agencies regarding treatment progress and referrals, as needed
Evaluates progress of treatment goals and adapts treatment, as needed
Attends multidisciplinary team meetings as needed, comprised of members from various referral and funding sources
Oversees referrals for medication for opioid use disorder (MOUD) and provides community based referrals for these services
Completes level of care and/or bio-psycho-social assessments with clients, including meeting with clients in hospital to complete assessments
Coordinates with family members for support and family therapy, as appropriate
Facilitates group therapy sessions
Provides individual therapy
Provides crisis intervention services
Provides integrated case management services
Maintains complete record of client's treatment
Administers substance testing throughout course of treatment
PROGRAM COORDINATOR CORE RESPONSIBILITIES
Develops programming for outpatient services (adult, youth and family services, jail/CCOP, and RSS) in consultation with Clinical Director
Provides 24/7 on-call support for outpatient staff
Hires and oversees training of new outpatient staff
Provides regular supervision, facilitates regularly scheduled staff meetings, and completes annual evaluations for outpatient staff and interns
Acts as part of the Dawn Farm administrative team by participating in meetings and activities
Compiles and maintains program data and other information for program evaluation
Oversees compliance with licensing, accreditation, and contract requirements, as applicable, in consultation with Project Manager
Addresses complaints, as needed
Maintains relationships with other community providers
Participates in all required trainings, staff inservices, and special events
Completes other duties, as requested by Clinical Director or President

Familiar with recovery principles and the philosophy and practice of 12-step recovery
Pursues continuing education to improve job knowledge and maintain licensing/certification, as applicable

SUPERVISORY RESPONSIBILITIES
Supervises outpatient staff (adult outpatient, youth and family, jail/CCOP, and RSS)
CREDENTIALS
Master’s degree from an accredited program in social work or a related field (per LARA: “a licensed counselor, LMSW, or licensed psychologist”)
CAADC or plan for certification
CCS or plan for certification
CPR certification
RECOVERY
At least eighteen months of continuous sobriety, if in recovery from addiction
If ever a client in any Dawn Farm program, at least one year since the date of last service
