Human Resources department should receive the following from all new hires:
Completed and signed Employment Application
Documentation of reference checks
Documented criminal background check (State of Michigan ICHAT system) If background check is found to have criminal history, Clinical Director or President will review background information and make final decision to hire. Completed I-9
Verification of credentials, when required by individual job description
Signed Receipt & Acknowledgment of Dawn Farm’s Employee Manual and policies within
Submit driver’s license information to our insurance agent to confirm that the new-hire is eligible to drive company-owned vehicles, drive personal vehicles for business purposes.
CADC application will also be provided to staff whose position requires it upon hire
Human Resources will notify the "Project Manager" of New hire, providing new hire's Name, official Start date, telephone number and emergency contact information.
Project Manager will set up Dawn Farm email, Relias and any other pertinent application logins for New Hire.
Project Manager will send a "Welcome Email" to New Hire providing them with application links, temporary passwords and other important initial information.
Personnel Change Form stating the expected number of hours to be worked and the initial rate of pay will be submitted by Program Coordinator
Completed W-4 and MI-W4 will be completed and returned by the employee

Credentials and licensing information check
The employee will provide documentation of the credential/certification, either upon hire or as the credential is obtained
Credentials and / or licensing information will be checked via the applicable source which may include:
LARA
MCBAP
National Provider Identifier (NPI) database
During the annual performance evaluation process, the employee will state all credentials/certifications and the supervisor will verify them
Verification will be documented in the employee’s personnel file
Dawn Farm expects and encourages employees to pursue continuing education to improve job knowledge and obtain/maintain certifications, as applicable. All employees who hold degrees that can be licensed should be working toward obtaining and maintaining that license.
