- DF Medical Director (for Spera clients)
- Notify your Program Coordinator and Project Manager if you give away a box of Narcan from the medication closet so that it can be replaced. You can do this by submitting an incident report via Apricot.
All programs ask clients during their intake and discharge planning if they are interested in a script for Narcan. Each site has specific steps for obtaining their Narcan, see below.
SPERA-
- When prompted in the Spera Intake, ask the client if they would like to have a prescription for Narcan
- If yes - document on the intake and send DF Medical Director an email with the client’s initials and Apricot # in the subject line and indication that this is for a Narcan Rx (Example: J.S. #45643 Narcan Rx)
- In the email to Medical Director, include a link to the clients Apricot file, their birthdate, and address/location of where they are for treatment (Spera) and that they would like a Narcan Rx
- If no - document on the intake and make a note of the reason they are declining
- DF Medical Director will then send the Rx into Hometown Pharmacy for delivery
- Once the Rx is delivered, notify client, make a progress note, and add to their belongings page + store in med basket to take with them at discharge
- If the client discharges against staff advice and indicates a plan to use, give them a box of our Narcan from the med closet and email Spera Coordinator and Project Manager (or complete an incident report).
TRANSITIONAL HOUSING-
- If you are discharging a resident who is at risk of opioid overdose (meaning they recently relapsed on opioids and/or are likely to use opioids in the near future), you can provide them with a box containing two doses of Narcan.
- Email Housing Coordinator and Project Manager (or complete an incident report) to let us know you provided it. New Narcan will be sent over
- Get the use form to Spera at some point within a week so that it can be sent to Project Manager via interoffice mail.
- If a client is not at imminent risk of overdose upon discharge and would like to have a supply of Narcan, direct them to any local pharmacy. The State of Michigan has a standing order to disperse Narcan with no prescription needed.
- - Typically the co-pay is as follows:
Medicaid: $0-$3 copay
Range $0-149 pending on your insurance
FARM/DFD-
- During discharge planning, ask clients if they are interested in leaving with Narcan.
- If yes, direct them to any local pharmacy. The State of Michigan has a standing order to disperse Narcan with no prescription needed.
- - Typically the co-pay is as follows:
- Medicaid: $0-$3 copay
- Range $0-149 pending on your insurance
- Make sure to document on Apricot discharge that client was given information about obtaining Narcan.
