DAWN FARM SUB-ACUTE DETOXIFICATION
GOALS, OBJECTIVES, ACTIVITIES
Goal #1
A safe, controlled environment will be made available for all community members to begin withdrawal from mood-altering sub stances and begin the process of recovery.
Objective 1.1 - Each Detox client should receive basic assessment, intervention, medical and social support services over a three to five day stay.
Activities The program will document the following activities in each client chart:
• Emergency housing and physical support during withdrawal.
• Medical review of symptoms, plan for care.
• Non-medical oversight of addiction-related health issues.
• Educational and social interventions
• Exposure to the local recovering community.
Objective 1.2 - Each Detox client should receive assessment of need for additional services that will assist in stabilizing his/her life.
Activities The program will document assessment and recommendations in the following areas for each client:
• Additional addiction treatment (if needed)
• Short-term and long-term housing
• Vocational services
Objective 1.3 - Each Detox client should receive the opportunity to provide feedback on services and customer satisfaction.
Activities The program will annually document measures to promote client input in the following:
• General client satisfaction with treatment, aftercare and referral
• Family/referral satisfaction with treatment, aftercare and referral
• Improvement in physical plant and services
• Additional client needs
Goal #2
The Dawn Farm Sub-Acute Detoxification Program will provide effective care to a cross-section of the community.
Objective 2.1 - The Detox program should be responsive to changing community needs, paying special attention to issues of client accessibility.
Activities The program will document evaluation of admission demographics, providing annual documentation of program response to the following:
• Maintenance of minority admissions to reflect community demographics.
• Maintenance of female admissions to reflect community demographics.
• Level of program response to homelessness, poverty and substance abused.
• Assessment of recidivism in order to maintain 70% one-time visit.
Objective 2.2 - The program should annually assess its criteria and methodology for referring clients elsewhere due to medical issues.
Activities The program will document the internal review of Triage criteria by Detox medical and clinical staff.
Goal #3
The Dawn Farm Sub-Acute Detoxification Program will provide services to the Washtenaw County Community that are measured and demonstrate effectiveness.
Objective 3.1 - The program should assure that the community effectively utilizes Detox services.
Activities • The program will document 45% utilization of available program beds.
• The program will annually document the assessment and evaluation of ASA departures with a plan for increasing completion rates to 75%.
Objective 3.2 - The program should demonstrate client success following discharge from detoxification.
Activities The program will document the internal evaluation of discharge and follow-up materials, evaluating the following:
• 50% successful engagement in other forms of treatment
• 10% abstinent from drugs and alcohol

Goal #1

• Basic housing and physical support during resident stay.
• Medical, clinical and psychosocial assessments, plan for care and referral.
• Non-medical oversight of addiction-related health issues.
• Instruction/education regarding substance abuse-related issues.
• Individualized treatment planning, therapy program and monthly review of progress.
• Exposure to the local recovering community.

Goal #2
Dawn Farm will provide effective care to a cross-section of our community.
Objective 2.1 - The Dawn Farm residential program should be responsive to changing community needs, paying special attention to issues of client accessibility.
Activities The program will document evaluation of admission demographics, providing annual documentation of program response to the following:
• Maintenance of admissions to reflect community demographics.
• Monitor admission demographics including homelessness, poverty and specific substances used (i.e. IVDU) and other Social Determinants of Health.
• Program Outcomes (see Discharge and Referral Categories)
• Include and prioritize individuals who are pregnant and experiencing addiction.
The Dawn Farm residential program will provide services to our community that are measured and demonstrate effectiveness.
Objective 3.1 - The program should assure that the community effectively utilizes residential services.
Activities
• The program will maintain utilization of 35 funded beds.
• The program will maintain utilization of 5 scholarship beds.
• The program will document and evaluate Chooses No Further Treatment (CNFT) discharges with a target of less than 20%.
• The program will document and evaluate Transferred Within Dawn Farm Continuum of Care discharges with a target of greater than 50%.
Objective 3.2 - The program should demonstrate client success following discharge from residential care.
Activities The program will document the internal evaluation of discharge and follow-up materials, evaluating the following:
• Reliable follow-up data at 3 months, 1 year, 2 years and 5 years comparing for Discharge and Referral Status.
• Trends in retention, completion and positive outcomes.
• Monitor the following standards for client success during follow up.:
a. Abstinence from alcohol and other drugs (AOD)
b. Active participation in mutual aid
c. Other recovery capital indicators.
Objective 3.3 - The program should demonstrate efforts to increase client retention.
Activities The program will document the internal evaluation of factors that might predict client departures/ discharges, and program/staff activities that are linked to these factors. The following will be evaluated:
• Themes identified in weekly Discharge reviews.
• Themes identified in Chart Reviews
• Ongoing assessment of client expectations.

Goal #3
The Dawn Farm Outpatient Services program will provide services to the community that are measured and demonstrate effectiveness.
Objective 3.1 - The program should assure that the community effectively utilizes outpatient services.
Activities
• The program will document 50% utilization of available client capacity.
• The program will annually document the assessment and evaluation of individuals who fail to complete outpatient services or are discharged.
• The program will document case-finding activities that encourage high risk clients to utilize Dawn Farm residential services.
Objective 3.2 - The program should demonstrate client success following discharge from outpatient services.
Activities The program will document the internal evaluation of discharge and follow-up materials, evaluating the following:
• Reliable follow-up data at 3 months and 1 year.
• Trends in client retention, completion and positive outcomes.
• Perceived role of age, drug of choice, race, gender in client success.
• Maintenance of 50% of graduates satisfying standards for client success after 1 year, as follows:
a. Abstinence from drugs and alcohol
b. Active participation in self-help (AA and/or NA)
• Maintenance of 30% of all discharges abstinent from drugs and alcohol.
Objective 3.3 - The program should demonstrate program flexibility in determining optimum number of outpatient visits.
Activities The program will document the internal evaluation of factors that might predict the need for shorter or longer treatment and types of services offered (e.g. group vs. individual), and program policy development that is linked to these factors. The following will be annually evaluated:
• follow-up data (as noted above).
• differing client expectations for appropriate length of treatment.
• perceived role of age, drug of choice, race, gender in length of stay.
• structure for greater client involvement in decision-making.
Objective 3.4 - The program should demonstrate efforts to increase client retention.
Activities The program will document the internal evaluation of factors that might predict client withdrawals/ discharges, and program/staff activities that are linked to these factors. The following will be annually evaluated:
• follow-up data (as noted above).
• assessment of client expectations in initial 30 - 60 days of care.
DAWN FARM WOMEN’S TRANSITIONAL SERVICES

Goal #1
An array of flexible services will be made available to support pregnant addicts and chemically dependent custodial mother in developing chemical-free life styles.
Objective 1.1 - Each WTP program participant should receive basic assessment, intervention, therapy. medical and social support services, as well as specialized services to support parenting needs.
Activities The program will document the following activities in each client chart:
• All services pertinent to residential treatment clients.
• Prenatal care and education
• Child care services
• Parenting classes/groups
• Gender specific groups
• Transitional Housing
• Transportation to therapy, doctors, etc.
• Specialized medical services
• Seminars (health, parenting)
• Client Advocacy for mothers and children

The Dawn Farm Women’s Transitional Program will provide effective care to a cross-section of women in the Washtenaw County Community.
Objective 2.1 - The Women’s Transitional Program should be responsive to changing community needs, paying special attention to special issues of accessibility for pregnant addicts and women with young children.
Activities The program will document evaluation of admission demographics, providing annual documentation of program response to the following:
• Maintenance of minority admissions to reflect community demographics.
• Level of program response to homelessness, poverty and specific abuse of substances (i.e. IVDA).
• Ease of access for pregnant addicts and custodial mothers.
Objective 2.2 - The Women’s Transitional Program should seek the input from a variety of community voices in established treatment activities.
Activities
• The program will document input from a diverse group of community leaders with expertise and/ or interest in the needs of pregnant addicts and chemically dependent custodial mothers.
Goal #3
The Dawn Farm Women’s Transitional Program will provide services to the Washtenaw County Community that are measured and demonstrate effectiveness.
Objective 3.1 - The program should assure that the community effectively utilizes WTP services. Activities The program will submit an annual performance report that assesses:
• Utilization of Transitional Services.
• Infants born drug-free while mothers participated in WTP.
• ASA departures and discharges.
• Case-finding and collaboration activities that encourage high risk women to utilize WTP services.
Objective 3.2 - The program should demonstrate client success following discharge from WTP.
Activities The program will document the internal evaluation of discharge and follow-up materials, evaluating the following:
• reliable follow-up data at regular intervals.
• trends in retention, completion and positive outcomes.
• perceived role of age, drug of choice, race in client success.
• maintenance of 70% of WTP graduates satisfying standards for client success after 1 year, as follows: a. Abstinence from drugs and alcohol
b. Active participation in self-help (AA and/or NA)
c. No legal involvement
d. Involvement in constructive life activity (work/school)
e. Evidence of improved parenting skills
DAWN FARM JAIL OUTREACH SERVICES

• Clinical and psychosocial assessments (as needed), plan for care and referral.
• Instruction/education regarding substance abuse-related issues.
• Individualized treatment planning, therapy program and monthly review of progress.

The Dawn Farm Jail Outreach will assess CCAB referrals to this program, to measure level of care to a cross-section of the Washtenaw County Community.
Objective 2.1 - The Dawn Farm Jail Outreach program should be responsive to changing community needs, paying special attention to issues of client accessibility.
Activities The program will document evaluation of admission demographics, providing annual documentation of the following:
• Minority admissions.
• Female admissions.
• Level of program response to specific abuse of substances (i.e. IVDA).
• Impact of program response to specific age groups.
• Impact of program response to populations with other special needs.

The Dawn Farm Jail Outreach will provide services to the Washtenaw County Community that are measured and demonstrate effectiveness.

Activities The program will document the internal evaluation of discharge and follow-up materials, evaluating the following:
• Admission to additional treatment (and type)
• Progress in treatment, completion of same
• Participation in aftercare/ongoing AA/NA support
• Legal involvement following program participation
• Evaluation of graduates for success following successful completion of Jail Outreach, as follows: a. Abstinence from drugs and alcohol

• Transitional Housing, room and board
• Assistance with transportation to work, AA, etc.
• Referral to medical services as needed
• Advocacy for other basic services, as necessary

Activities The program will document evaluation of admission demographics, providing annual documentation of program response to the following:
• Maintenance of minority admissions to reflect community demographics.
• Level of program response to homelessness, poverty and specific abuse of substances (i.e. IVDA).
• Ease of access for homeless addicts and alcoholics.
Objective 2.2 - The Chapin Street Project should seek the input from a variety of community voices in establishing program activities.
Activities
• The program will document the establishment of community input on services in the Chapin Street Project, soliciting input from community representatives who have expertise and/or interest in the needs of homeless addicts and alcoholics.

The Dawn Farm Chapin Street Project will provide services to the Washtenaw County Community that are measured and demonstrate effectiveness.

RECOVERY IS EVERYWHERE

Goal #1
To reduce the stigma associated with drug addiction, normalize recovery and offer hope to those affected by addiction.
Objective 1.1 - We will develop advertising materials that promote recovery and challenge common stereotypes of who addicts are
Activities
• Seek input from a diverse cross-section of the recovering community on images and messages that they believe represent them.
• Develop images and messages that present addicts and alcoholics as normal people.
• Develop images and messages that show successful people in recovery.
Objective 1.2 - We will challenge the commonly held assumption that addicts only harm their communities.
Activities
• Develop images and messages that present recovering addicts as contributing family and community members.
• Develop images and messages that present recovering addicts as professionals
Goal #2
To assist recovering people in replicating the campaign in their communities.
Objective 2.1 - We will share materials with people across the United States
Activities
• Print materials that can be mailed to other advocates/recovering people
• Share print ads and other images so that they may be printed or published in other communities.
Objective 2.2 - We will provide assistance for partners in other communities.
Activities
• Assist partners in preparing images for publication and other advertising mediums.
• Develop and distribute materials to assist partners in obtaining donated ad space.
Goal #3
To assure that the campaign is used only to reduce stigma, normalize recovery and offer hope to people suffering from addiction.
Objective 3.1 - Our materials will not be used to promote treatment or other commercial or institutional interests
Activities
• We will not identify Dawn Farm on any materials.
• We will require all partners to agree to our terms of use
• We will monitor use of the materials.
