SOP for St. Vincent De Paul

Goal – why is this important?

Helps client’s with limited funds to obtain clothing – ie: Outreach client needs clothing for employment

Ingredients – what do I need to get started?

Release of Information, Name, Date of Birth, Letter Head, SVD Referral Template

Timeframe – about how long will this take?

Paperwork – approximately 5 minutes

Transport client to SDV to obtain clothing – approximately 1 hour

Contacts – who do I need to let know?

Inform the client that they have received a voucher and schedule a time to go shopping together. Inform Program Coordinator if client is in Spera or Residential services.

Steps – what do I do?

1. Have client sign Release of Information

2. Locate SVD Referral Letter Template

3. Add client information to letter template

4. Print letter on Dawn Farm letterhead

5. Sign letter

6. Make a copy of the letter to put into the client file

7. Place original copy of letter into an envelope and give it to the client

8. Ask client if he/she needs transportation to SVD

9. Add to client Tx Plan (if applicable)

10. Write RSS note
