CONTRIBUTION TO MISSION & CONTINUUM OF CARE
The goal of Dawn Farm is to remove the barriers that people with addictions face to entering into recovery. Therapists at Dawn Farm further this mission by________________________________________
CORE RESPONSIBILITIES
Completes level of care (LOC) and/or bio-psycho-social assessments with clients
Develops individual recovery plans with clients
Facilitates group therapy sessions
Provides individual therapy
Provides crisis intervention services
Provides integrated case management services
Maintains complete record of client's treatment
Administers substance testing throughout course of treatment
Interacts with other community agencies regarding treatment progress and referrals, as needed
Assists and trains new employees and interns
Monitor funding for treatment services (self pay record, public-funding, etc)
Maintains and updates LOC recommendation options
Conducts MOUD assessments and referrals
Monitors waitlists and assessment times for compliance
Audits outpatient treatment files as directed by the Outpatient Coordinator
Completes other duties, as requested by program coordinator
Completes and documents emergency drills and site inspections
Participates in all required trainings, staff meeting, inservices, and special events
PERSONAL CHARACTERISTICS
ALL DAWN FARM EMPLOYEE RESPONSIBILITIES
Integrates Dawn Farm mission and core values into daily functions
Familiar with recovery principles and the philosophy and practice of 12-step recovery
Pursues continuing education to improve job knowledge and maintain licensing/certification, as applicable
Able to interact with a wide variety of people in a cordial, professional manner, both independently and as part of a team
Able to interact with clients and family members in an empathetic manner while maintaining professional boundaries
SUPERVISORY RESPONSIBILITIES
Takes the lead on training new outpatient staff
Facilitates outpatient staff meetings when the Outpatient Coordinator is not present
Provides coaching and guidance for outpatient staff
CREDENTIALS
Master’s degree in Social Work or related field strongly preferred
CAADC or plan for certification
CPR certification
RECOVERY
At least eighteen months of continuous sobriety, if in recovery from addiction.
If ever a client in any Dawn Farm program, at least one year since the date of last service.
