CONTRIBUTION TO MISSION & CONTINUUM OF CARE
The goal of Dawn Farm is to remove the barriers that people with addictions face to entering into recovery. The Dawn Farm Intake Team helps to further this mission by acting as a “front door” to all programs- providing information and completing screenings for all potential clients and their families.
CORE RESPONSIBILITIES
Answers phones and provides information to interested parties about Dawn Farm services and admissions procedures
Completes initial screening for services and contacts supervisors and/or medical staff as appropriate for guidance
Facilitates client referrals, including funding sources, hospitals, treatment providers, etc.
Schedules appointments and manages waitlists
Acts as part of the treatment team, documenting and communicating all relevant information to other staff
Proficiency in various computer applications including Google Suite, electronic medical records etc.
Performs various clerical tasks, including scanning, making copies, etc.
Participates in staff meetings, required trainings, staff inservices, and special events
Completes other duties, as requested
Completes and documents emergency drills and site inspections
Assists with training of new staff

Familiar with recovery principles and the philosophy and practice of 12-step recovery
Pursues continuing education to improve job knowledge and maintain licensing/certification, as applicable

Able to interact with clients and family members in an empathetic manner while maintaining professional boundaries

CREDENTIALS
Bachelor's Degree in social work or related field (strongly preferred)
CPR certification
RECOVERY
At least eighteen months of continuous sobriety, if in recovery from addiction.
If ever a client in any Dawn Farm program, at least one year since the date of last service.
