CONTRIBUTION TO MISSION & CONTINUUM OF CARE
The goal of Dawn Farm is to remove the barriers that people with addictions face to entering into recovery. The Transitional Housing Coordinator at Dawn Farm furthers this mission by overseeing supervision of transitional housing sites and by supporting clients and House Managers through recovery housing and helping connect them to the recovering community.
CORE RESPONSIBILITIES
- Screens Transitional Housing applicants for eligibility
- Provides information to prospective clients about Transitional Housing
- Consults with Dawn Farm Intake Team and other treatment providers to coordinate intake of new clients into Transitional Housing
- Identifies community resources that support recovery and links clients as needed
- Maintains office hours at 2208 Packard for employment and other assistance
- Recruits and maintains partnerships for Recovery is Good Business project
- Oversees billing for transitional housing sites
PROGRAM COORDINATOR CORE RESPONSIBILITIES
- Develops programming for Transitional Housing in consultation with Clinical Director and President
- Provides 24/7 on-call support for transitional housing
- Hires and oversees training of new House Managers
- Provides regular supervision and annual evaluations for House Managers
- Acts as part of the Dawn Farm administrative team by participating in admin meetings and activities
- Compiles and maintains program data and other information for program evaluation
- Ensures Transitional Housing compliance with all health and safety policies
- Addresses complaints, as needed
- Maintains relationships with other community providers
- Participates in all required trainings, staff inservices, and special events
- Completes other duties, as requested by Clinical Director or President
PERSONAL CHARACTERISTICS

- Integrates Dawn Farm mission and core values into daily functions
- Familiar with recovery principles and the philosophy and practice of 12-step recovery
- Pursues continuing education to improve job knowledge and maintain licensing/certification, as applicable
- Able to interact with a wide variety of people in a cordial, professional manner, both independently and as part of a team
- Able to interact with clients and family members in an empathetic manner while maintaining professional boundaries
SUPERVISORY RESPONSIBILITIES
- Supervises team of House Managers
CREDENTIALS
- Bachelor's degree from an accredited program in social work or a related field strongly preferred
- CPR certification
RECOVERY
- At least eighteen months of continuous sobriety, if in recovery from addiction.
- If ever a client in any Dawn Farm program, at least one year since the date of last service.

- This position regularly coordinates moving of transitional sites from one location to another, which may include lifting up to 50 pounds.
