Discharge Procedures Specific to the Farm, DFD, & Spera:
Staff and client will discuss progress and/or completion of treatment goals.
Together, staff and client will develop an Aftercare plan. Staff will be provided referrals for aftercare or continuing care, as appropriate. Client will receive a copy of this plan.
The client will be instructed to pack their belongings. Dawn Farm is not responsible for any personal belongings. Personal items left in storage must be picked up within 72 hours. If the client does not pick up items, they will be donated off site. A staff member must be present while items are gathered for donation.
All appropriate persons (CMHPSM, family, Washtenaw County Jail, Probation Officer, etc.) are notified of client discharge.
Client will be asked to fill out a Discharge Client Satisfaction Questionnaire
Staff will document the departure in the Daybook and client file.
Staff will complete the discharge. At Spera, the discharge will be completed immediately. At the Farm or DFD, the primary therapist will close the case file within two weeks.
Follow up will be made with each client within 72 hours.
When Dawn Farm does not appear able to meet a client's needs or the client is failing to progress in treatment the following additional procedures are followed:
Decisions should be made with input from all available staff, and with consultation from a supervisor.
Referrals for alternative services, from Dawn Farm or other providers, are provided. This may involve direct placement in another facility, especially when there is a psychiatric disorder present.
Client may use the phone to arrange transportation and may remain on premises if behavior is not disruptive.
At the Farm or DFD, the client will be instructed to pack their belongings with the help of two other residents. In the event that a client is not present, staff will assist along with 2 other residents to pack the clients belongings. At Spera, the client will pack their belongings with a staff member present.
Outpatient
Discharge Procedures Specific to Outpatient
Staff and client will discuss progress and/or completion of treatment goals.
Together, staff and client will develop an Aftercare plan. Client will receive a copy of this plan.
Staff will provide referrals for continuing care, if appropriate.
Client will be asked to fill out a Discharge Client Satisfaction Questionnaire.
All relevant and previously agreed upon parties (CMHPSM, family, Washtenaw County Jail, Probation Officer, etc.) are notified of client status.
Staff will document the departure in the client file.
Staff will complete the discharge. For Outpatient, the discharge summary will be completed within 2 weeks.
When Dawn Farm Outpatient does not appear able to meet a client's needs or the client is failing to progress in treatment the following procedures are followed:
