Initial follow up & incident reporting about 15-20 minutes.
If a detailed plan is required for follow up, this may take several hours or days. A plan for staffing during this time may be necessary.

The Project Manager will contact the Medical Director for any further assistance.
If a client has been diagnosed with a communicable disease:
1. Confirm the diagnosis with the clinic. Ask for a copy of the diagnosis to be faxed over or just a make a copy of the diagnosis if the client presents it to you. Request a release for lab work.
2. Ask the clinic the following questions:
3. Contact the Project Manager with the diagnosis and the answers to the above questions asap. If Project Manager is unavailable, contact the Clinical Director. If neither are available, contact our Medical Director directly. Document the answers in the incident report.
4. The Project Manager will draft a response plan* and send the plan along with the above information immediately to the Medical Director & Clinical Director for approval. If urgent, both call and email Medical Director with information.
5. Once the plan is approved, the Project Manager will distribute information and instructions to staff and clients as deemed necessary. This should include a WCHD Fact sheet print out if available.
All care must be taken to keep the confidentiality of the client who received the diagnosis.
Fill out an Incident Report & make sure to complete the Communicable Disease section.
*Sample Response Plan (A snapshot of what you might see)
1.Ask client to share diagnosis with anyone they have had close skin to skin contact with or shared bedding/clothing items with. Staff ensure that all those in close contact are aware without sharing client identifying information.
2. Staff/house managers use gloves to launder bedding and towel items in hot soapy water. Items should be dried on high heat.
3. Staff/house managers use gloves to vacuum any carpeted areas, then dispose of vacuumed dust.
4. Using gloves, material Items that can’t be laundered can be put in a ziploc bag or in outside area for at least 3 days.
5. If client is able, make an appointment with a dermatologist for further care.
