CONTRIBUTION TO MISSION & CONTINUUM OF CARE
The goal of Dawn Farm is to remove the barriers that people with addictions face to entering into recovery. Dawn Farm Jail and CCOP staff further this mission by providing substance use treatment services to clients that are currently incarcerated or participating in Community Corrections Outpatient programming. Jail and CCOP staff also create aftercare plans with clients to reduce relapse or recidivism risk.
CORE RESPONSIBILITIES
Facilitates group therapy sessions
Provides crisis intervention services
Provides integrated case management services
Maintains complete record of client's treatment
Completes referrals for substance testing throughout course of treatment
Interacts with other community agencies regarding treatment progress and referrals, as needed
Completes jail and CCOP level of care assessments
Coordinates treatment resources for higher levels of care, as needed
Connects clients with other community resources for transportation, food, housing, etc. as needed
Participates in all required trainings, staff inservices, and special events
Completes other duties, as requested by program coordinators
PERSONAL CHARACTERISTICS
ALL DAWN FARM EMPLOYEE RESPONSIBILITIES
Integrates Dawn Farm mission and core values into daily functions
Familiar with recovery principles and the philosophy and practice of 12-step recovery
Pursues continuing education to improve job knowledge and maintain licensing/certification, as applicable
Able to interact with a wide variety of people in a cordial, professional manner, both independently and as part of a team
Able to interact with clients and family members in an empathetic manner while maintaining professional boundaries
SUPERVISORY RESPONSIBILITIES
None
CREDENTIALS
Master’s degree in Social Work or related field strongly preferred
CAADC or plan for certification
CPR certification
Clearance to work in the Washtenaw County Jail
RECOVERY
At least eighteen months of continuous sobriety, if in recovery from addiction
If ever a client in any Dawn Farm program, at least one year since the date of last service
