Clients may need additional medical care from time to time during treatment, and we want to make sure they are able to get to the hospital.

Write a contact in the client’s file that they are going to the hospital.
- Farm and DFD staff should write a note in the daybook and call on-call staff.
Call U of M (or St. Joes for Farm clients) Social Work and inform them that we are sending over a client.
Have the client sign all necessary UM guideline/ROI paperwork, give the client a copy and put a copy in their file or their Primary’s mailbox.

If it asks for a code you will need to log into the uber health gmail account.

The code should be in the top most recently received email.
Then it will have you input the "rider info" which would be the name of the client we are sending.
Uber is HIPPA compliant but we're being cautious and will put the FIRST name of the client and the LAST name will always be "Spera" or “DFD” or “Farm”.
Enter your facility phone number, pick up location, and drop-off location and BAM - we've got an UberHealth cab on the way!
Enter a note to the driver with instructions for pickup of client- ask that the driver call when they arrive so that we don't miss them
Uber may send updates via email about the status of the cab. You can check for emails by using the same uber@dawnfarm.org account and password (ubercab2) in gmail.
Spera- Call U of M social work once per shift to check the status of client and write a contact with any updates.
Farm and DFD- Call on-call staff for further instructions
**Remember- U of M Social Work should be arranging transportation back from the hospital for our clients. You may need to speak with the Social Work department to coordinate when a client is ready to discharge from hospital.
