Individuals seeking admission to Dawn Farm Residential must meet the following criteria:
- Age 17 or older
- Be fully detoxified and chemical-free (detoxification services are available)
- Be free of warrants for their arrest
- Be motivated and willing to participate in program
- Be free of symptoms of severe and persistent mental illness
- Not be on a sex offender registry list
- Not have an unpaid balance to Dawn Farm for services provided. (Publicly funded referrals are exempt from this, though they are ineligible for admission to transitional housing after residential.)
Potential clients are required to call the Dawn Farm Intake Team (DFIT) and complete a prescreen. If a bed is available, the Intake Team will schedule an intake. If a bed is not available, the client will be placed on a waitlist. (See waitlist procedure page for further information)
- DFIT will call to schedule an intake interview when a bed becomes available if the client was placed on a waitlist.
- Once the client arrives for their intake, the President or Clinical Director will complete the intake interview. The intake interview includes:
- Demographic information
- Potential sources for support
- Emergency contact information
- A mental status screening
- Medical information including preferred doctor and hospital
- Legal information
- DSM-V assessment for substance dependence
- Substance use history
- Psych and previous substance abuse treatment history
- Employment and educational history
After the intake interview is complete, clients receive and sign a Dawn Farm Client Handbook, complete releases of information, satisfaction survey and informed consent
If a client is refused admission for any reason, he/she will be given other resources for treatment, either within the Dawn Farm continuum or to another provider.
